How to Submit the Awards Luncheon Form
1. First, submit payment for the luncheon.
a. If you do not submit payment, your form submission WILL NOT be
considered.
b. The Award Luncheon is $45.00 per person. NO INVOICING!
c. To submit payment, visit the PAB Award Luncheon Checkout and follow the steps to pay online.
2. Next, complete the Awards Luncheon Form. This form is meant to complete the details of your purchase.
b. Complete all questions on all pages. The form is complete when you see the message: “Your responses were successfully submitted.”
